Faktor-Faktor Yang Menyebabkan Duplikasi Nomor Rekam Medis Di Puskesmas Wisma Indah Bojonegoro

Abstract

Background :The numbering system is the provision of medical record numbers for patiens on treatment at healt services. Numbering system numbering system is the provision of one medical record number for outpatiet patients and patients for hospitalization, the number provided will be used forever for continous visits and the media record will be stored in one file with the same number. Duplication of medical record numbers in the storage room of the puskesmas indah Bojonegoro in the medical record file is presented with 6% of medical record file. This study aims to determine the factors causing duplication of medical record number from the registration process, numbering system, storage systems and duplication events at the indah Bojonegoro public health center. Subjects and Method :Deskriptive research with observation and interview method of population and sample in this study are 2 people registration officers and 2 people in the filling section responsible medical records with the sampling technique used in Purposive Sampling. Results :Based on the results of the research the numbering system used is numbering system (UNS) and centralized storage. The result of duplication is 6%. Conclusion :In the manual registration process, the numbering system used is the numbering system (UNS) unit, the storage system used is centralization.
